In order to best serve residents whose homes were damaged or destroyed by the Marshall Fire, the City of Louisville's Planning and Building Department is working to form a clear picture of the experience and plans of residents in relation to insurance, demolition and rebuilding. We would be grateful if you could take a couple of minutes to complete this short survey. We understand that you may not have complete answers to many of the questions, but please just answer to the best of your ability with regard to your current situation and plans. Registration is not required for this survey and answers are anonymous.

This survey is intended for residents, who have been given the opportunity to opt-in or opt-out of the Coordinated Debris Removal Program.
